Acceptance

By accessing or using Cupula Health, Inc. (“Cupula”) services — including our application portal, physician dashboards, and public directory — you agree to these Terms. If you don't agree, don't use the service.

Eligibility (physicians)

Physician accounts are available only to licensed clinicians operating a Direct Primary Care practice. You represent that the information in your application is accurate, including your NPI, license status, and practice details. We reserve the right to verify and to decline or revoke access at our discretion.

Not medical advice

Cupula is a directory and marketplace. We do not provide medical advice, diagnosis, or treatment, and we are not a covered entity or business associate under HIPAA unless a separate BAA is executed in writing. All care is provided by independent physicians, not by Cupula.

Acceptable use

• No misrepresentation of credentials or practice.
• No scraping, reverse engineering, or automated access without written permission.
• No unlawful, harassing, or infringing content.

Your content

You retain ownership of the practice information, bios, and media you submit. You grant Cupula a non-exclusive, royalty-free license to host, display, and distribute that content for the purpose of operating the directory.

Termination

You can close your account at any time by emailing hello@cupula.health. We may suspend or terminate access if you violate these Terms or if required by law.

Disclaimers & limitation of liability

The service is provided “as is” without warranties of any kind. To the maximum extent permitted by law, Cupula is not liable for indirect, incidental, or consequential damages, and our aggregate liability is limited to the amounts you paid us in the preceding twelve months (or $100 if none).

Changes

We may update these Terms. Material changes will be announced via email or an in-product notice. Continued use after the effective date constitutes acceptance.
